This section addresses any students with allergies to medication, food or insects. Please review the forms in this section and complete any necessary forms. If medications need to be given, please refer to the medication section and complete the forms as necessary. Parents should also provide any emergency medications needed. 

 

OSH 34: this form gives a thorough assessment of the child's allergies, history and medications use to treat in case of emergency.
