100 Neediest Cases is a holiday assistance program sponsored by United Way which serves to help families get what they need during the holiday season. Please start the application process by clicking on the following link https://forms.office.com/r/kZQAA2a16x and I will follow up with you for next steps.  Please note that your application is not submitted until I have all the required information/documents and you hear from me. Forms are due September 8th, 2023.  In order to receive assistance, you will be asked to provide some sensitive information while also meeting the below requirements:

☐  Proof of Income

☐  Proof Client Meets 125% of the Federal Poverty Guidelines

Addtionally, I will need documentation of at least two (2) unmet needs.  Such documentation may include: $500+ in past due utility bills or disconnection notice; medical bills/medical diagnosis; past due rent or eviction notice; unemployment check or change of work status; educational expenses such as tuition; documentation of being unhoused; social security/disability letter; or other.
