To register, please call the main office at (314) 867-0634 to make an appointment. The following items are needed in order to register your child: 

• ID (Driver's license)
• Proof of Residency (i.e. Unpaid utility bill) 
• Original Birth Certificate 
• Immunization Record
• Physical Exam
• Dental Exam
• Report Card (If coming from another school) 

If you do not have the documents listed, or have any questions, please call the main office and someone will be glad to assist you. 

You will be asked to complete the forms below at the time of registration. If you prefer, you can print and complete them prior to your appointment.
