    • Parent or Guardians valid state identification 
    • Parent or guardians' proof of address (lease or deed) one current utility bill
    • Students Birth Certificate
    • Students Social Security Card
    • Updated Shot Record, Physical or any medical information for the nurse
    • Previous schools report card 
    • IEP or Recourse information
