Hello Wilkinson Families!
My name is Nurse Bell, and I am the School Nurse at Wilkinson ECC. I am very grateful I get to be a part of your children's lives and keeping them safe and healthy while at school. I graduated from the University of Missouri-St. Louis in 2014 with a Bachelor's of Science in Nursing. After nursing school, I worked at Mercy South on their Surgical Stepdown floor as an RN. I have 5 children who all attend SLPS schools. I love spending time with my big family and being outside. Required Enrollment Paperwork

To enroll at Wilkinson, the following must be turned in to the main office or school nurse;
- Physical Exam (dated within the last year)
- Immunization Record
- Dental Exam (Pre-K only)
- Lead and Hemoglobin Test Results (Pre-K only)
The following should also be completed and turned in to the main office or school nurse, but is not required for enrollment;
- Authorization for Emergency Care
- Over-the-Counter Medication Administration
- Student Health Registration Form
If your student needs to take any type of medication at school, the following form needs to be completed;
- Authorization for Administering Medication
- Medication will need to be brought to school by a caregiver, including inhalers and EpiPens. Medications cannot be brought to school in the student's backpack.
*If your student has asthma, life-threatening food allergies, seizures, or sickle cell, an action plan will need to be completed by your physician and turned in.*
- All forms can be found under the tab "Student Health Forms." If using a phone to view this page, click the "+" sign near my name and a drop-down menu will appear showing "Student Health Forms"
- Physical and Dental forms can be printed off and turned in from the physician, it does not need to be on the district form.
