Request for leave must be made at least thirty (30) days prior to the date the requested leave is to begin, or as soon as practicable under the circumstances. Please contact the District’s Employee Relations Department at erstaff@slps.org for detailed instructions on how to apply for a leave of absence.
The various types of leave of absence are listed below:
- Americans with Disabilities Act (ADA)
• Own Serious Health Condition
• On the Job Accommodations
- Family and Medical Leave (FMLA)
• Own Serious Health Condition
• Birth of Child
• Placement of Child for Adoption or Foster Care
• Care for an Immediate Family Member (Spouse, Child, Parent)
• Sabbatical Leave
• Military Leave
• Spouse, Child, Parent is on active duty and notified of impending call to active duty in support of a contingency operation
• Care for Spouse, Child, Parent or next of Kin who is a covered service member recovering from a serious illness/injury in the line of duty on active military duty
• Educational Leave with Pay
• Educational Leave without Pay
• Bereavement Leave
• Jury Duty
- Leave of Absence and Pay:
If an employee has an accrued bank remaining (i.e., sick, PTO, vacation), then they may choose to be paid using the accrued bank which should be indicated to their timekeeper and the Employee Relations Department. An employee may apply for STD-LTD if they meet the requirements for eligibility following a 30-day waiting period which begins on the first date of leave. Short-Term is handled by CIGNA who can be reached at 1-800-362-4462. If approved, the employee would receive 60% of their pay for the timeframe approved by CIGNA following their process. CIGNA would work with employees to transition to Long-Term Disability should the need arise.
